Topic 1: Interpretation of Abnormal Liver Function Tests – When to Refer to a Specialist?
Objective: Equip general practitioners (GPs) with a systematic approach to interpreting abnormal liver function test (LFT) results and identifying cases requiring specialist referral.
Session Outline:

  1. Case Presentation (5 mins):
    A 50-year-old male with fatigue and abnormal ALT and AST.
  2. Lecture (15 mins):
    • Overview of key liver function markers (ALT, AST, ALP, GGT, Bilirubin, Albumin, INR).
    • Common patterns and their implications (Hepatocellular vs. Cholestatic vs. Synthetic dysfunction).
    • When to monitor vs. when to refer: Red flag signs (persistent elevation, jaundice, INR abnormalities).
    • Approach to abnormal LFTs in asymptomatic patients.
  3. Q&A and Discussion (10 mins).

Topic 2: Application of FIB-4 Scoring – When to Send for Fibroscan?
Objective: Guide GPs on using FIB-4 scoring for liver fibrosis assessment and determining when further assessment via Fibroscan is warranted.
Session Outline:

  1. Case Presentation (5 mins):
    A 55-year-old diabetic patient with mild transaminitis and metabolic syndrome.
  2. Lecture (15 mins):
    • Introduction to FIB-4 scoring (Age, AST, ALT, Platelets).
    • Cut-off values for risk stratification and limitations of FIB-4.
    • When to escalate to Fibroscan (intermediate to high-risk patients).
    • Role of Fibroscan in chronic liver disease management.
  3. Q&A and Discussion (10 mins).

Topic 3: Common Liver Ultrasound Scan Findings – What to Do Next?
Objective: Help GPs interpret common liver ultrasound findings and decide on appropriate management steps.
Session Outline:

  1. Case Presentation (5 mins):
    A 60-year-old woman with an incidental liver lesion detected on ultrasound.
  2. Lecture (15 mins):
    • Common benign and malignant liver findings (fatty liver, simple cysts, hemangioma, HCC).
    • Key ultrasound features differentiating benign vs. suspicious lesions.
    • When to order additional imaging (CT/MRI) or refer to hepatology.
    • Role of follow-up scans and monitoring strategies.
  3. Q&A and Discussion (10 mins).

Topic 4: Understanding Hepatitis B Serology Test Results – What to Do if a Complete Course of HBV Vaccine Does Not Work?
Objective: Enhance GPs’ understanding of Hepatitis B serology and provide guidance on managing non-responders to HBV vaccination.
Session Outline:

  1. Case Presentation (5 mins):
    A 40-year-old healthcare worker with no response to HBV vaccination.
  2. Lecture (15 mins):
    • Interpreting HBV serology (HBsAg, Anti-HBs, Anti-HBc).
    • Understanding vaccine response failure (primary non-response vs. waning immunity).
    • Management options:
    – Revaccination strategies (high-dose, intradermal, alternative schedules).
    – Checking for chronic HBV infection or immunosuppression.
    – Post-exposure prophylaxis considerations for healthcare workers
